Recreating the Breast Mound

During the first step of reconstruction, Dr. Hermann will coordinate with your mastectomy surgeon, oncologist, and any other doctors to create a reconstructive plan to rebuild the breast mound once damaged tissue has been removed due to injury or breast cancer. This collaborative effort is essential in planning a successful surgery following treatment for breast cancer and other issues.

Although breast reconstruction can be performed immediately following a mastectomy, an oncologist may recommend that you undergo the procedure at a later date. Dr. Hermann can perform a delayed procedure or provide revision surgeries for patients who are unhappy with results of a previous procedure.

The Breast Reconstruction Procedure

When planning surgery, Dr. Hermann uses a tool called BodyLogic. This software allows patients to virtually preview their reconstructed breasts and explore the results that are possible with different types and sizes of implants.

Dr. Hermann uses fat grafting and AlloDerm regenerative tissue matrix to perform breast reconstruction. He primarily works with a mastectomy surgeon specializing in skin- and nipple-sparing mastectomy, a procedure that retains the skin envelope of the breast and the nipple in order to achieve highly natural-looking results.

The initial procedure can take six to seven hours if performed immediately after a mastectomy. Before surgery, you will be sedated with general anesthesia. After the breast tissue has been removed, Dr. Hermann can begin performing fat grafting or the insertion of a breast implant or tissue expander.

Breast Tissue Expanders vs. Flap Surgery

When performing breast reconstruction, Dr. Hermann chiefly uses tissue expander devices to create room for a permanent implant. He prefers this method rather than another reconstructive procedure called flap surgery. Flap surgery involves transferring a flap of tissue from another part of the body to create the breast mound. Flap surgery is a very complex procedure and there is a chance the flap may not survive if a complication with its blood supply occurs.

Meanwhile, an expander is an inflatable implant that stretches the breast muscle and skin over time. It is placed beneath the skin and pectoral muscle immediately following your mastectomy. Dr. Hermman will fill the implant with saline over time. When the skin has stretched enough, the expander will be removed and replaced with a permanent implant during a second surgical procedure. Dr. Hermann can discuss the kind of implant that will work best for you during your consultation.

Breast Reconstruction Recovery

Immediately following breast reconstruction surgery, you will wake up in recovery. Antibiotics, pain medication, and other necessary fluids to promote healing and comfort will be provided through an IV.

Surgical drains will also be placed near each incision. These will collect fluid in order to reduce swelling in the surgical areas. The drains are generally removed about one to two weeks after the procedure.

You will be provided with a specialized compression bra to help your implant settle and reduce swelling.

Please follow Dr. Hermann's instructions regarding cleaning surgical wounds, wearing your compression bra, changing bandages, and keeping drainage tubes clean. Take it easy until Dr. Hermann says it is OK to exercise.

You may experience temporary side effects such as soreness, bruising, swelling, and discomfort, but these will all fade over the next few weeks.

In most cases, it takes about six to eight weeks before you can resume all normal activity. However, the timeline is different for each patient because everyone heals at different rates.

Tell Me About Breast Reconstruction Scarring

As with any surgery, scars are to be expected. Women who have undergone breast reconstruction can expect to have scars in one or more areas, including the breast and nipple.

Although these will fade significantly over time, they will never disappear completely. However, Dr. Hermann can minimize the appearance of scarring with strategic incision placement that can be determined during your consultation.
